ii. Mobile crisis teams

A.   Mobile crisis team members adequately trained to address the crisis shall respond to individuals at their homes and in other community settings and offer timely assessment, services, support, and treatment to de-escalate crises without removing individuals from their current placement whenever possible.

B.   Mobile crisis teams shall assist with crisis planning and identifying strategies for preventing future crises and may also provide enhanced short-term capacity within an individual’s home or other community setting.

C.   Mobile crisis team members adequately trained to address the crisis also shall work with law enforcement personnel to respond if an individual with ID/DD comes into contact with law enforcement.

D.   Mobile crisis teams shall be available 24 hours per day, 7 days per week and to respond on-site to crises.


About Mobile Crisis Teams

Upon receiving crisis call and determining an individual is in crisis the REACH staff will respond in person. Upon arrival, the REACH staff will:

·         assess the situation to offer suggestions for de-escalation of immediate crisis and safety strategies based on observation to stabilize the person in the immediate situation,

·         evaluate the need for mobile supports or other intensive community supports,

·         collaborate with ES/ED staff to assess need for hospitalization (if clinically applicable),

·         explain what REACH services can offer

Mobile Crisis services are intended to be collaborative in nature and require the system supporting the individual to actively participate when the REACH Mobile team is there.  Mobile crisis is delivered through coaching and mentoring not only the individual but also the people who support that individual.
